Oregon OSHA administers among the highest US penalties: serious $1,214–$17,004; serious-fatality $21,765–$54,412; willful/repeat $12,147–$170,046; willful/repeat fatality up to $272,058; failure to correct up to $15,625/day. Three willful/repeats or a fatality triggers mandatory wall-to-wall inspections.
